New Passport Checklist
Passport applications are FREE through the Department of State's official website at travel.state.gov.
Applicants should complete the following New Passport Checklist. Missing information will delay passport processing.
NOTE: Current estimated passport processing times are:
Routine: 6 - 8 weeks
Expedited: 2 - 3 weeks
These do not include mailing times. For a more accurate estimate, please add at least another week.
Checklist
☐ Complete DS-11 passport application. This form should be completed in black ink and have no white-out or scribbling. Do not sign the application until meeting with our passport services staff.
☐ Appear in person. All ages. Children under 16 years old must appear in person to apply for a passport.
☐ Present valid Missouri State Driver License or valid government ID.
☐ Present proof of U.S. Citizenship (original birth certificate, U.S. Passport or original naturalization certificate).
☐ Provide one passport photo. Passport photo services are available at Library passport acceptance locations for a $15 fee.
☐ Pay two separate fees to the U.S. Dept. of State and to the Library. If paying by check, two checks are required.
☐ Pay the required fees to the U.S. Dept. of State (check or money order only). Families may write one check or money order to the U.S. Department of State for their passport application fees if they are applying in person at the same time. For all other applicants, separate checks must be made for each application. See Fees section.
☐ Pay the required fees to the Library.
Passport Renewals
- For those looking to renew their passports, the Library can take and print your photo, attach it to your application, and check the application for any errors.
- The Library is not allowed to send in passport renewal applications.
- Please note that you can now renew your passport online with the U.S. Department of State.

Execution Fee
The Library must collect an execution fee for each passport application processed.
- $35 per passport application processed.
- May be paid with debit/credit card, cash, check, or money order made out to the St. Charles City-County Library.
All personal checks must have a name and address pre-printed on them.
Photo Fees
- $15 per photo
- May be paid with debit/credit card, cash, check or money order made out to the St. Charles City-County Library.

Other Information
- Missouri law regulates notaries. We will not notarize documents that do not follow such laws and regulations. Notaries will not prepare or complete documents for you, nor can they give legal advice on a matter.
- Per Missouri law, notaries will not provide service if the document or circumstances of the request for notary service raise any question of authenticity, ambiguity, doubt, or uncertainty for the library. In this event, the notary may decline to provide notary service at their sole discretion.
